RESERVATION | Fire Academy of KY

This form is to be completed by a department Chief, Training Officer, or designee of a Kentucky Fire Department (Career or Volunteer). Reservations received from other individuals will not be considered. This is the initial step for a department to reserve spots in the upcoming Recruit Class. This is not an official enrollment of an individual recruit(s). This helps the Academy staff plan for an expected number of recruits based on reservations received. Please reserve only the number of spots that you are expecting to hire/send to the Recruit Class. We understand that departments may hire lateral transfers, or have applicants unable to complete the hiring process, and that is ok. The reservation can be updated after submission by contacting us directly. Upon receipt of your reservation request, we will communicate directly with those listed with further instructions, information, and how to enroll each individual with the Recruit Class at a later date.

I confirm that each recruit is a member of a Kentucky fire department, holds a current (issued within last 12 months) CPAT card, and is physically prepared to participate in daily physical exercise and strenous hands-on training. I understand that a financial reimbursement agreement will be signed by the department during enrollment, and reimbursement will be required for any recruit who is expelled, removed, or resigns from the Recruit Class.
